Registration Information

  • All applicants must be pre-registered.
  • Each applicant must fully complete (including all signatures) the application form. If additional pages are required, photocopy the original form only. NO SUBSITUTE
    FORMS WILL BE ACCEPTED.
  • Please PRINT in BLOCK LETTERS or TYPE on the form. And make sure your Instructor has signed the document. All non-legible applications will be rejected without notice.
  • Each applicant must submit one cheque for their registration fee and one cheque for
    their certificate fee. If the applicant fails to pass his/her examination, their certificate
    fee will be returned to them or destroyed.
  • A representative of each dojo will submit all individual application forms (and
    individual cheques) as a group to their respective regional representatives. Individual applications submitted on their own will not be accepted.
  • All Applications must be received by Jeff Nadeau by No later than June 21, 2008. Late registrations will not be accepted.

Techniques, kata, and Tachi uchi no kurai as determined by the examiners.  Demonstration of teaching techniques.
